Contact Form
With the contact form, you can ask us questions or make requests.

For this, we use your (company) name, phone number, and email address. We do this based on your consent. We retain this information until we are certain that you are satisfied with our response and for a maximum duration of 6 months thereafter. This way, we can easily refer to the information when there are follow-up questions.

PSA-index BV collaborates with Open Health Hub BV for the processing of this data from your questionnaire. This partner is certified according to ISO 27.001 and NEN 7510. We retain your information in accordance with the legal obligation for a duration of 15 years.

Scientific Research
:
Only if you have explicitly given consent after logging in, we use your data for scientific research. Of course, your data will be used in anonymized form for this purpose.

Group Report for Your Employer
:
Your employer can request PSA-index BV to provide a group report. Your employer uses this information to establish and adjust its occupational health and safety policy. If your employer has requested a group report from PSA-index BV, we use your data for this purpose in an anonymized manner. We do not provide separate reports for groups smaller than 15 individuals.

Provision to other companies or institutions
Except for the aforementioned partners, we do not provide your personal data to other companies or institutions under any circumstances, unless required by law. With companies that may process your data on our behalf, we enter into a data processing agreement to ensure the same level of security and confidentiality of your data. PSA-index BV remains responsible for these processing activities.
